Meet
Wayne
Wayne, 56, has a vision impairment, ongoing mental health challenges, and a history of substance use. For years, he struggled to survive while sleeping rough on the streets, unable to access stable housing or consistent support.
His disability made navigating services even harder, leaving him isolated and without direction. Despite these barriers, Wayne was determined to find stability and regain control over his life.

Meets
Circle
Wayne’s path to change began when Circle SC stepped in to help. We connected him with Urban Fabric, an organization providing diverse and affordable housing solutions.
With our guidance, Wayne secured stable accommodation tailored to his needs, giving him a safe foundation to rebuild his life.
Beyond housing, we supported Wayne in applying for an NDIS plan that meet his disability support needs including his vision impairment, mental health support, and therapy.
Wayne now has:
- Regular support workers to assist with daily tasks
- Therapy services focused on mental health and recovery
- A dedicated team ensuring he stays on track with his goals
- Regular connection and review from health and mental health services
By working closely with Wayne throughout this process, we ensured he received the holistic, long-term support he needed to thrive.

Wayne
Today
Today, Wayne is no longer living on the streets. He has a safe, secure home and a structured routine that includes therapy, support work, and personal development.
With stability in his life, he can now focus on his mental health, recovery, and long-term well-being.
Wayne’s journey highlights the power of coordinated holistic support—NDIS, housing, healthcare, and the community working together to create real change.
I have a vision impairment and other complex needs but my previous support coordinator and workers didn’t get me the right supports so I was homeless and almost lost everything. My support worker put me in touch with Circle SC who have been amazing.
Emma came and met with us initially even when there was no funding. She has helped me get back on track with housing, psychological services, a travel card and a companion card.
I was homeless and Circle CS helped me get transitional housing in a boarding house till my accommodation came through. I’ve had really bad experiences with my previous Support Coordinators so I have difficulty trusting people again. But Emma has been amazing.
